You are agreeing to let your minor child engage in potentially dangerous activities. Even where T3 Youth Development exercises reasonable care, there is a chance your child may be seriously injured or killed because there are certain dangers inherent in these activities which cannot be avoided or eliminated.

By submitting this form, you are giving up your child's right and your own right to recover from T3 Youth Development in a lawsuit for any personal injury, including death, or property damage resulting from risks that are a natural part of the activity.

This waiver does not apply to gross negligence or intentional misconduct. You have the right to refuse to submit this form.

  • Falls, trips, slips, and collisions with objects or other participants
  • Dehydration, heat exhaustion, heat stroke, and effects of extreme heat or humidity
  • Traffic, vehicular hazards, and uneven or unpaved running surfaces
  • Exposure to wildlife, insects, and environmental hazards
  • Exposure to infectious diseases, viruses, and communicable illnesses
  • Equipment malfunction or failure
  • Risks associated with adult mentoring, group transit, and community service environments
